New Patients: please arrive ten minutes before your appointment to fill out new patient paperwork.
- Active duty do not require a referral and may contact the PT clinic directly.
- All non-active duty require a referral to make an initial appointment.
- Follow-up appointments and treatment appointments will be scheduled based on individual patient needs.

Physical Therapy Customer Evaluation (ICE)
Patients are encouraged to complete the online Physical Therapy Customer Service Survey each time you visit our facility, especially if you observe a difference in the care received, whether positive or negative. Feedback from our dental patients is extremely important in assisting our team to make a positive and lasting impression of our facility and your care.
